What Is It?

Breast augmentation increases the size, fullness, and projection of the breasts using implants or fat transfer. It is one of the most commonly performed plastic surgeries worldwide. Implant-based augmentation uses silicone or saline-filled prostheses, while fat transfer uses the patient's own fat for a more modest, natural enhancement.

How It Works

For implant augmentation, an incision is made in the inframammary fold (under the breast), periareolar area, or axilla. The implant is placed either behind the breast tissue (subglandular) or behind the pectoral muscle (submuscular or dual-plane). Fat transfer involves liposuction of a donor site and injection into the breast tissue.

Realistic Results

Implants: immediately visible increase in breast size and fullness. Fat transfer: a more modest 1–1.5 cup size increase with a natural feel, requiring a second session in some cases. Implant results last many years but implants typically require revision or replacement after 10–20 years.

Recovery

Implants: sports bra worn 24/7 for 4–6 weeks, no overhead lifting for 2 weeks, return to desk work in 5–7 days, exercise restricted for 4–6 weeks. Fat transfer: compression garment at donor site for 4 weeks, less restriction on breast activity.

Risks & Complications

  • !Implants: capsular contracture (scar tissue around implant)
  • !Implants: implant rupture (silicone gel or saline leakage)
  • !Implants: breast implant-associated ALCL (BIA-ALCL) with textured implants (very rare)
  • !Implant: malposition or displacement
  • !Fat: unpredictable survival and potential need for revision
  • !Both: changes in nipple sensation

Red Flags — Choose a Clinic Safely

  • Surgeon does not discuss implant type, surface texture, and the associated ALCL risk
  • No discussion of pocket placement (sub vs. subglandular) and why one is recommended
  • Surgeon cannot explain mammogram implications with implants
  • No discussion of long-term implant management including potential future replacement

Questions to Ask Before Booking

  • What implant size, profile, and surface texture do you recommend for my anatomy?
  • Should my implant be placed in front of or behind the muscle, and why?
  • What is the risk of capsular contracture with the implant you are recommending?
  • How will implants affect future breast cancer screening?
  • What is your plan for implant management in 10–20 years?
