What Is It?
Breast implant removal (explantation) involves surgically removing breast implants that are no longer wanted, have ruptured, or have caused complications such as capsular contracture. Some patients choose removal and immediate replacement with new implants; others opt for removal alone, sometimes with a breast lift.
How It Works
The surgeon accesses the implant through the original scar. The capsule (scar tissue around the implant) can be left, partially removed (partial capsulectomy), or completely removed (total or en bloc capsulectomy). The decision depends on the condition of the implant and whether BIA-ALCL is a concern.
Realistic Results
Removal of the implants restores the breast to its natural state, though the breasts will appear smaller and often saggier than before augmentation. Capsular contracture symptoms resolve. If a lift is combined, the breast shape is improved.
Recovery
Sports bra for 4–6 weeks. Return to desk work in 5–7 days. Full activity in 4–6 weeks. Recovery is typically shorter than the original augmentation.
Risks & Complications
- !Scar tissue (capsule) may require additional surgery to remove
- !Breast ptosis (sagging) after removal
- !Asymmetry in the remaining breast tissue
- !Infection if capsule is infected at removal
- !Rarely: BIA-ALCL cells in textured implant capsules requiring careful pathological assessment
Red Flags — Choose a Clinic Safely
- ⚑Surgeon does not discuss the condition of the capsule or whether it needs to be removed
- ⚑No mention of sending the capsule for pathological examination in patients with textured implants
- ⚑Surgeon dismisses the patient's concern about systemic symptoms without referral
- ⚑No discussion of what the breasts will look like after removal
Questions to Ask Before Booking
- Should I have an en bloc capsulectomy, or is partial capsule removal appropriate?
- Will the removed capsule be sent for pathological analysis?
- Should I combine removal with a breast lift?
- Will there be enough tissue remaining for a natural-looking breast after removal?
- If I want new implants in the future, is that straightforward?
