What Is It?

Buccal fat removal is a surgical procedure that extracts the buccal fat pads — small pockets of fat in the mid-cheek area — to create a slimmer, more sculpted lower face with more visible cheekbone definition. It is best suited to adults with naturally round, full cheeks who are at a stable weight.

How It Works

Small incisions are made inside the mouth in the upper gum area, near the second and third molars. The buccal fat pad is gently identified, teased out, and partially or fully removed. The incisions are closed with absorbable sutures. The procedure takes 30–60 minutes under local anaesthesia.

Realistic Results

The mid-cheek thins, the cheekbone ridge appears more defined, and the face takes on a more sculpted, angular appearance. Results are permanent and become progressively more apparent as residual swelling resolves over 2–3 months.

Recovery

A soft diet is required for 1–2 weeks. Mild swelling and tightness are expected for 2–4 weeks. Oral hygiene must be maintained carefully. Most patients return to work in 3–5 days.

Risks & Complications

  • !Over-removal leading to a gaunt or hollow appearance in later years as natural facial fat continues to reduce with age
  • !Damage to the parotid duct or facial nerve branches (rare)
  • !Infection inside the mouth
  • !Asymmetry between the two sides
  • !Dryness of the mouth or changes in salivation

Red Flags — Choose a Clinic Safely

  • Surgeon does not warn that natural facial fat loss with age may make results look too hollow in 10–20 years
  • No discussion of whether your round face is due to buccal fat or overall facial adiposity
  • Surgeon recommends full removal of the fat pad (partial removal is generally safer)
  • Clinic targets very young patients who have not yet lost youthful baby fat

Questions to Ask Before Booking

  • Am I a suitable candidate given my age, facial structure, and long-term fat loss predictions?
  • Will you remove the entire fat pad or a portion — and what is your reasoning?
  • How does this procedure interact with natural facial ageing over time?
  • What does over-removal look like, and can it be corrected?
  • Is this combined with any other procedure to address overall facial shape?
