What Is It?
Forehead reduction surgery (hairline lowering) lowers the frontal hairline to reduce a proportionately large or high forehead. It is suited to patients with a straight, low-hairline and good hair density. The procedure brings the hairline closer to the brow, creating a more balanced facial third.
How It Works
An incision is made at the front edge of the hairline, following the natural hairline contour. A strip of forehead skin is excised, and the scalp is advanced downward and sutured. Hair follicles at the incision edge are carefully preserved so hair grows through and in front of the scar over time.
Realistic Results
The hairline can be lowered by 1–3 cm in a single procedure. Final results, including full scar coverage by hair, are visible at 6–12 months. The scar becomes virtually invisible as hair grows through it.
Recovery
Swelling of the forehead and around the eyes is most pronounced in the first week. The scalp feels tight for 2–4 weeks. Sutures or staples are removed at 10–14 days. Most patients return to work in 7–10 days.
Risks & Complications
- !Visible scar if hair does not grow through the incision line
- !Temporary loss of sensation across the upper forehead
- !Hair follicle loss at the incision line
- !Limited downward movement if scalp laxity is insufficient
- !Tightness or headaches during healing
Red Flags — Choose a Clinic Safely
- ⚑Surgeon does not assess scalp laxity before determining how far the hairline can be lowered
- ⚑No discussion of natural hairline shape and whether it can be replicated
- ⚑Clinic recommends forehead reduction when a hair transplant alone would achieve the same result
- ⚑Surgeon cannot explain how hair will grow through the incision scar
Questions to Ask Before Booking
- Do I have sufficient scalp laxity to lower my hairline by my target amount?
- How do you ensure the scar is hidden within the hairline?
- Is my natural hairline shape (straight, M-shaped, curved) replicated?
- Would a hair transplant achieve the same effect without surgery?
- What is the maximum achievable reduction for my anatomy?
