What Is It?
Jaw reduction surgery reshapes the lower jaw to create a softer, more oval or heart-shaped facial outline. It is most commonly sought to reduce a wide or square mandible that creates a hyper-masculine appearance. The procedure involves removing or reshaping the outer bony cortex of the mandibular angle.
How It Works
Through incisions inside the mouth at the rear molars, the surgeon accesses the mandibular angle. An oscillating saw or burr is used to reduce the outer plate of bone or reshape the angle entirely. In some patients, the masseter muscle is also reduced (through muscle relaxant injections or surgical thinning) to achieve the full effect.
Realistic Results
The lower face narrows and the jaw takes on a more tapered or feminine appearance. Final results are fully visible at 3–6 months once swelling has resolved. Results are permanent as bone does not regenerate.
Recovery
A liquid and soft diet is required for 2–4 weeks. Significant swelling of the lower face is expected for 4–8 weeks, with the jaw feeling tight and numb. Most patients return to work in 2–3 weeks. Full resolution of swelling can take 3–6 months.
Risks & Complications
- !Temporary or permanent altered sensation in the lower lip and chin
- !Asymmetry of the jaw angle
- !Damage to the inferior alveolar nerve
- !Haematoma inside the mouth
- !Infection requiring antibiotics or drainage
- !Over-reduction creating a sharp or unnatural jaw contour
Red Flags — Choose a Clinic Safely
- ⚑Surgeon does not perform 3D CT imaging or cephalometric analysis before planning surgery
- ⚑No discussion of the inferior alveolar nerve position
- ⚑Clinic discourages Botox masseter reduction as a non-surgical alternative for borderline cases
- ⚑Surgeon cannot show 3D simulation of expected results
- ⚑Provider with no craniofacial or maxillofacial credentials offers the procedure
Questions to Ask Before Booking
- Do I need 3D CT imaging, and will you use computer planning to guide the bone reduction?
- How much bone will be removed, and will the masseter be addressed as well?
- What are the risks to the nerves in my lower jaw?
- How long until I can eat normally?
- What does over-reduction look like, and how is it prevented?
