What Is It?
Penile lengthening surgery releases the suspensory ligament that attaches the base of the penis to the pubic bone, allowing the internal portion of the penis to extend externally. It can add 1–2 cm of flaccid length, though erect length improvement is variable and often minimal.
How It Works
A small incision is made at the base of the penis at the pubic hairline. The suspensory ligament is divided, releasing the penile base. Weights, stretching, or a spacer device may be used postoperatively to maintain the gained length during healing. Penile fat removal (lipoplasty) at the base is sometimes combined.
Realistic Results
Average gain in flaccid length of 1–2 cm. Erect length gain is often less predictable and may be minimal. The pubic fat pad removal (if performed) can improve the visual appearance of penile length without changing actual size.
Recovery
Return to sexual activity after 4–6 weeks. A penile stretching programme is required for several weeks post-surgery. Swelling and bruising for 1–2 weeks.
