What Is It?

Perineoplasty reconstructs and tightens the perineum — the area between the vaginal opening and the anus — which may have become lax, scarred, or distorted following childbirth, episiotomy, or perineal tearing. It improves both the appearance and function of the perineum and posterior vaginal opening.

How It Works

Under local or general anaesthesia, old scar tissue is excised, the perineal body is reconstructed by suturing the perineal muscles together, and the vaginal entrance is tightened. The posterior vaginal wall may also be repaired (posterior colporrhaphy) if laxity extends into the vaginal canal.

Realistic Results

Improved perineal support, a normalised vaginal entrance size, and correction of perineal deformity. Most patients report improved function and comfort. Full healing takes 6–8 weeks.

Recovery

A high-fibre diet and stool softeners are essential to prevent straining at the wound. No sexual intercourse for 6–8 weeks. Swelling and discomfort last 1–2 weeks. Return to desk work in 3–5 days.

Risks & Complications

  • !Wound dehiscence if the patient strains during bowel movements
  • !Infection at the repair site
  • !Overcorrection creating a narrowed vaginal opening
  • !Scarring causing discomfort
  • !Recurrence of laxity after subsequent childbirth

Red Flags — Choose a Clinic Safely

  • Surgeon does not assess the full pelvic floor before recommending a perineal repair
  • No discussion of stool softeners and bowel management in the recovery plan
  • No gynaecological or urogynae assessment before surgery
  • Surgeon cannot describe which specific structures will be repaired

Questions to Ask Before Booking

  • Will you repair the perineal muscles, the posterior vaginal wall, or both?
  • How do I prevent wound breakdown during the recovery?
  • Are there functional improvements beyond appearance I can expect?
  • Will this affect my ability to deliver vaginally in the future?
  • Is pelvic floor physiotherapy recommended before or after surgery?
