What Is It?

Peyronie's disease causes fibrous scar tissue (plaques) to form within the penis, resulting in curvature, shortening, or pain during erection. Surgical correction is considered for stable disease (unchanged for 12 months) that causes significant functional difficulty. Techniques include plication (straightening), grafting, or penile implant placement.

How It Works

Nesbit plication shortens the unaffected side to correct curvature without touching the scar. Incision and grafting releases the plaque and fills the resulting defect with a graft, straightening the penis while preserving length. Penile implant placement is the preferred approach when there is concurrent erectile dysfunction.

Realistic Results

Curvature correction to within 15–20 degrees of straight in most patients. Results are permanent, though natural erectile function may be affected depending on the technique. Pain during erection resolves in the vast majority of patients.

Recovery

No sexual activity for 4–8 weeks. Swelling and bruising for 2–3 weeks. Erectile function should be tested gently at 6–8 weeks post-operatively.

Risks & Complications

  • !Recurrence of curvature
  • !Shortening of the penis (particularly with plication technique)
  • !Erectiledysfunction post-operatively
  • !Changes in penile sensation
  • !Graft site complications (if grafting used)

Red Flags — Choose a Clinic Safely

  • Surgery recommended for active, changing disease (surgery should wait for stability)
  • No imaging (ultrasound or MRI) to assess plaque location before planning surgery
  • Surgeon cannot explain the difference between plication and grafting and which is appropriate
  • No discussion of the effect on erect length after surgery

Questions to Ask Before Booking

  • Has my disease been stable for at least 12 months?
  • Which surgical technique is appropriate for my degree of curvature — plication or grafting?
  • How will this affect my erect penile length?
  • What is the risk of new erectile dysfunction after surgery?
  • Do I need a penile implant given my existing erectile function?
