What Is It?
Tear trough filler is a precise hyaluronic acid injection technique used to soften the hollow depression that runs from the inner corner of the eye down along the lid-cheek junction. This hollow creates the appearance of dark circles, shadowing, and a tired look. Filler placed here can dramatically refresh the eye area.
How It Works
A thin, low-density hyaluronic acid filler is injected via a blunt cannula just above the orbital bone, within the tear trough groove. The cannula approach reduces the risk of bruising and vascular injury in this high-risk zone. The filler blends the lid-cheek junction and reduces shadowing.
Realistic Results
The under-eye area appears smoother, less hollow, and significantly less tired-looking. Results are immediate but best assessed at 2–4 weeks. Duration is 9–12 months, though some patients see results lasting longer as the deep plane is less mobile.
Recovery
This area bruises easily — patients should budget for 7–10 days of possible bruising. Swelling is usually mild and resolves in 3–5 days. Cold compresses help in the first 24 hours. Makeup can cover bruising after 24 hours.
Risks & Complications
- !Bruising is more common here than in other areas
- !Tyndall effect — a bluish discolouration if filler is placed too superficially
- !Vascular occlusion near the ophthalmic artery
- !Lumpiness if too much filler is injected or placed incorrectly
- !Oedema (fluid retention) in the filler if large volumes are used
Red Flags — Choose a Clinic Safely
- ⚑Provider uses a thick lip-volume filler rather than a thin, specific tear trough product
- ⚑Needle rather than cannula used without clear justification
- ⚑Very large volumes recommended in a single session
- ⚑Provider has not treated tear troughs before and cannot show prior results
- ⚑No discussion of whether dark circles are caused by hollowing, pigmentation, or vascularity (filler only helps hollowing)
Questions to Ask Before Booking
- What filler product and volume do you plan to use in the tear trough?
- Will you use a cannula, and why is that the safer approach here?
- Are my dark circles caused by hollowing (which filler addresses) or pigmentation (which it does not)?
- What is the risk of the Tyndall effect, and how do you prevent it?
- If a lump develops, how quickly can it be dissolved?
