What Is It?
Under-eye filler is the same as tear trough filler — a precise hyaluronic acid injection placed in the hollow below the eye to reduce the appearance of dark circles, shadowing, and fatigue. It is one of the most technically demanding filler treatments due to the thin, delicate tissue and proximity to vascular structures.
How It Works
Low-viscosity filler is injected via a fine cannula into the pre-periosteal plane along the orbital rim. The cannula is passed from a single entry point to deliver filler along the entire length of the tear trough with minimal trauma to the surrounding tissue.
Realistic Results
Significant improvement in under-eye hollowing and the tired appearance associated with it. Results last 9–18 months. Most patients are satisfied after a single session, though some prefer a small touch-up at 4 weeks once initial swelling has resolved.
Recovery
Bruising is the primary expected side effect and may be significant for 7–14 days. Sleeping with the head elevated helps reduce post-procedure oedema. Social activities can generally resume in 5–10 days with makeup coverage.
Risks & Complications
- !Bruising and periorbital oedema
- !Tyndall effect if filler is placed too superficially
- !Vascular occlusion with potential retinal artery involvement (rare but serious)
- !Lumping or nodules under the eye
- !Filler-related fluid oedema with excess product
Red Flags — Choose a Clinic Safely
- ⚑Provider injects more than 0.5ml per side in the first session
- ⚑No cannula used in this high-risk area
- ⚑Provider does not know the vascular anatomy of the orbit
- ⚑Hyaluronidase not immediately available
- ⚑Consultation does not assess whether the patient's concern is hollowing, pigmentation, or fat prolapse
Questions to Ask Before Booking
- How much filler do you plan to use, and will you be conservative given the risk of this area?
- Do you use a cannula exclusively for under-eye filler?
- Can you show me patients where you have corrected hollow under eyes?
- Is hyaluronidase available on-site in case of a complication?
- What should I do if I notice blurring of my vision or eye pain after leaving the clinic?
