What Is It?
Acne scarring occurs when inflamed acne lesions damage the dermis, causing either depressed (atrophic) scars — such as ice-pick, boxcar, and rolling scars — or raised (hypertrophic or keloid) scars. Treatment requires an assessment of scar subtype as different scars respond to different modalities.
How It Works
Atrophic scars are treated with a combination of subcision (releasing tethered scar tissue), filler, RF microneedling, and fractional laser resurfacing. Hypertrophic scars respond to steroid injection, laser, or surgical revision. A multi-modal approach targeting different aspects of the scar gives the best overall improvement.
Realistic Results
A 50–70% improvement in scar appearance is a realistic target with a comprehensive treatment programme. Ice-pick scars respond to TCA CROSS (chemical reconstruction of skin scars). Rolling scars respond best to subcision and filler. Boxcar scars improve with fractional laser.
Recovery
Varies by treatment: subcision and filler have 3–5 days of bruising; fractional laser has 5–7 days of redness and peeling; RF microneedling has 2–3 days of redness.
