What Is It?
Calf augmentation adds volume and definition to underdeveloped or asymmetric calf muscles using solid silicone implants or fat transfer. It is sought by patients whose calves remain thin despite exercise, those with asymmetry, or those reconstructing calf shape after injury or medical conditions.
How It Works
For implants, an incision is made in the natural crease behind the knee. A pocket is created beneath the fascia of the gastrocnemius muscle, and a solid silicone implant is placed and positioned. Fat transfer to the calf is also performed, though the calf is a technically demanding fat grafting site with lower survival rates.
Realistic Results
Fuller, more defined calf muscles with improved leg proportion. Implant results are permanent. Fat grafting results vary with 40–60% fat survival. Final results are apparent at 3–6 months.
Recovery
Walking is restricted for 2–3 weeks post-implant. The legs must be elevated frequently for the first 2 weeks. Return to full activity at 6 weeks. Compression stockings are worn during the healing period.
Risks & Complications
- !Implant: displacement, infection, or extrusion
- !Implant: pain or difficulty walking if placed incorrectly
- !Fat grafting: partial resorption and asymmetry
- !Both: limited range of ankle motion during healing
- !Scarring behind the knee
Red Flags — Choose a Clinic Safely
- ⚑Surgeon has not performed calf augmentation regularly — this is a less common procedure requiring specific experience
- ⚑No discussion of the safety profile of the implant used
- ⚑Surgeon does not discuss post-operative walking restrictions upfront
- ⚑Fat transfer offered without acknowledging the low survival rate at this site
Questions to Ask Before Booking
- How many calf augmentation procedures have you performed, and can I see results?
- Will you use implants or fat transfer, and why?
- How is the implant secured to prevent displacement?
- How long will walking be restricted after surgery?
- What does infection or implant extrusion look like, and how is it managed?
